Nurse Home Visit Programs

Nurse Support Program

The Nurse Support Program can provide nursing assessments and screenings for the entire family, health promotion, and education on topics like nutrition, breastfeeding, immunizations, birth control, and parenting. The Nurse Support Program also provides support with care coordination, referrals, and resources. This program is designed to support and help families stay together. The Public Health Nurse works directly with Douglas County Human Services but is not a case worker.

What will the nurse do to help my family? The nurse will work with your family to help you meet your goals for success with education, health and nutrition, and parenting. The focus of the home visits is based on the needs of your family. Services include:

  • Medical Assessments/Screening: Prenatal, post-partum, newborn and well-child assessments, tracking growth and development, and evaluation of needs for ongoing health issues.
  • Education/Promotion: Breastfeeding, parenting, safe sleep, diagnosis-specific education, healthy relationships, immunization education, finding a medical home, and nutrition, wellness, and goal setting.
  • Prevention/Support: Coordination with health care providers, perinatal-related depression and anxiety, child abuse and neglect, domestic violence, and substance use disorders.
  • Referrals/Resources: Help with food, housing, and medical resources, obtaining baby supplies, and referrals to other support services including Temporary Assistance to Needy Families (TANF), Supplemental Nutrition Assistance Program (SNAP), and Supplemental Nutrition Program for Women, Infants, and Children (WIC).

How often and where will I meet my nurse?  A nurse will typically visit your family at your home or a location that is convenient for you. The frequency and duration of visits are decided on by you and your nurse.

Nurse Family Partnership

Nurse Family Partnership (NFP) is a voluntary, community health home visitation program for women who are expecting their first baby. The program supports mothers of any age in having a healthy pregnancy and provides the education needed for successful parenting.

This program is currently supported by Tri-County Health Department until December 31, 2022, at which time the Arapahoe County Public Health will begin supporting Douglas County residents who are part of the program.
